The image haunts him and this is when his vertigo begins which has symptoms of aversion to heights. He takes early retirement and ends his days as a detective until an old college chum, Gavin, pleads for his help in a private case.
This is where the drama becomes intense. Gavin tells Scottie that his wife is infatuated with a dead woman, Carlotta Valdes. She spends a daily routine visiting the places that have some connection to Carlotta and Gavin fears that his wife is insane.
Scottie spends the day trailing Madeleine, Gavin's wife, and he witnesses her jump into the bay at Fort Point with the Golden Gate looming above her. After he rescues her there is a chemistry between them and Madeleine tells Scottie about her fears of death and a recurring dream with a bell tower in it.
They figure out that this landmark is in a small town, San Juan Bautista and Madeleine is convinced to face her fears and see if this can stop the nightmares. When they arrive she flees to the top of the tower. Scottie's vertigo will not allow him to climb higher. He hears a scream and witnesses a body land at the bottom.
He is beside himself with grief. Everyone he thinks he sees her. One day he meets a woman, Judy, who looks exactly like Madeleine. They form a relationship and he has her dress like Madeleine, dye her hair blond and go to the same places they went.
A twist arises when Carlotta Valdes' necklace is spotted on Judy. Scottie figures out that they are the same and he decides to confront her with the truth.
He revisits San Juan Bautista with Judy determined to get the truth. He finds out that they are the same woman. The whole thing was a plot to murder Gavin's first wife, the woman that actually fell from the bell tower.
They battle it out and a nun steps forward. Judy screams. She falls off the bell tower to her death below. Scottie's vertigo is no more.
